GUIDE or “Guiding an Improved Dementia Experience” Model is being piloted by the Centers for Medicare & Medicaid Services (CMS). It is an alternative payment model for dementia care that focuses on both the patient and their caregiver. It provides comprehensive, coordinated care through a dedicated Care Navigator and aims to improve outcomes, reduce costs, and increase health equity. Eligibility for beneficiaries is limited to those with traditional Medicare Parts A and B, that have a diagnosis of dementia. They must be patients of a participating provider to enroll, as they cannot self-enroll.
